orochimaru can use his hand as we have seen during his battle with 4 tail naruto, after the Sannin fight he said that there were other ways to get them back, which was by taking over another body, and he only waited so long because he was hoping to take sasukes otherwise he would have done it sooner.
I don't remember it being said or shown that your hands are needed to dispel a genjutsu. u need have a flood of chakra or stop it in order to dispel genjutsu and if you can't have someone infuse their chakra into your body.

Itachi is besides madara one of the most powerfull characters. I don't think he will have a hard time against orochimaru due to the sharingan.
